口服伊曲康唑胶囊治疗六例婴幼儿血管瘤疗效评价

摘要: 目的:报道6例口服伊曲康唑胶囊治疗婴幼儿血管瘤的患者,观察并分析其临床疗效及安全性。方法:收集我院确诊为婴幼儿血管瘤并接受伊曲康唑治疗的6例患儿,按照5 mg/(kg·d)的剂量给予患儿口服伊曲康唑胶囊,定期随访,使用血管瘤活跃度评分(HAS)评估病情,并参照经典Achauer分级的百分比阈值进行总体疗效评定,同时回顾国内外发表的有关伊曲康唑治疗婴幼儿血管瘤的文献。结果:本研究纳入的6例患儿中,男1例,女5例;其中浅表型5例,混合型1例;4例患儿瘤体位于四肢,2例泛发全身;初始治疗的年龄范围为1.67~4.27个月,有效患儿显效时间在6~16周,服药疗程范围为9~26周,最终5例治疗有效,总有效率达83.3%(5/6),期间定期复查血常规及肝功能均未见明显异常。结论:口服伊曲康唑胶囊治疗婴幼儿血管瘤有效,且临床耐受性良好,无明显不良反应。

Abstract: Objective: To evaluate the clinical efficacy and safety of oral itraconazole capsules in the treatment of infantile hemangioma (IH) based on a six-case series. Methods: A retrospective review was conducted on six infants diagnosed with IH who received oral itraconazole capsules at a dosage of 5 mg/(kg·d) at our institution. Patients were followed up regularly. Lesion changes before and after treatment were evaluated using the Hemangioma Activity Score (HAS). The HAS improvement index was calculated, and overall clinical efficacy was graded according to the percentage thresholds of the classic Achauer grading system. Furthermore, a comprehensive literature review regarding the use of itraconazole for IH was performed. Results: Six children were enrolled, including 1 male and 5 females; 5 cases were classified as superficial type, and 1 case as mixed type. The tumors were located on the extremities in 4 cases, and 2 cases presented with disseminated lesions. The age at the initiation of treatment ranged from 1.67 to 4.27 months. For the effective cases, the time to significant improvement ranged from 6 to 16 weeks, and the treatment duration ranged from 9 to 26 weeks. Ultimately, the treatment proved effective in 5 cases, yielding a total effective rate of 83.3% (5/6). Regular monitoring of blood routine and liver function during the treatment period revealed no significant abnormalities. Conclusion: Oral administration of itraconazole capsules represents an effective and well-tolerated therapeutic option for infantile hemangioma, with no notable adverse events observed.
