The role of Th17 related cytokines and chemokines in pemphigus

Abstract: Objective: To investigate the role of Th17-associated cytokines IL-6, IL-17, IL-21, IL-22, IL-23 and chemokines CXCL-8 and CCL-5 in the pathogenesis of pemphigus. Methods: The levels of Th17 related factors were detected by ELISA in patients with pemphigus before and after treatment, patients with bullous pemphigoid and healthy human controls. The relationships  of the severity of pemphigus and  Th17-associated cytokines expression levels in various subtype were analyzed. Results: Thirty one patients with pemphigus were collected including 18 cases of pemphigus vulgaris, 7 cases of pemphigus foliaceus and 6 cases of pemphigus erythematous. And 15 patients with bullous pemphigoid and 20 healthy were taken as controls. The levels of IL-6, IL-17, IL-21, IL-22, IL-23 and chemokines CXCL-8 and CCL-5 in pemphigus group were (32.31±7.98)pg/mL, (17.00±4.53)pg/mL, (491.13±72.41)pg/mL, (150.91±23.32)pg/mL, (264.89±67.84)pg/mL, (27.91±10.79)pg/mL, (47.21±17.83)pg/mL, which were significantly higher than those in healthy controls (8.36±1.96)pg/mL, (8.41±1.36)pg/mL, (372.73±18.45)pg/mL, (93.78±9.47)pg/mL, (163.17±9.48)pg/mL, (8.89±4.13)pg/mL, (22.66±2.85)pg/mL, with significant differences (Ps<0.001). After the treatment of the pemphigus patients, the level of IL-6, IL-17, IL-21, IL-22, IL-23 and chemokines CXCL-8, except for CCL-5, decreased (P<0.001). There was a significant difference of the IL-21 level in pemphigus group (491.13±72.41)pg/mL and bullous pemphigoid group (559.58±78.56), (P<0.05), and there were no significant difference of other cytokines (Ps>0.05). There was no significant differences of all the cytokines in the patients with pemphigus vulgaris, pemphigus foliaceus, pemphigus erythematous (Ps>0.05). There were correlations between the levels of IL-17, IL-21, IL-22, IL-23, CCL-5 and the severity of pemphigus (P<0.05).  Conclusion: Th17-associated cytokines and chemokines may be involved in the pathogenesis and associated with the severity of pemphigus.
