浅层X线联合曲安奈德注射治疗颌面部痤疮后瘢痕疙瘩疗效评价

Assessment on superficial X-ray therapy combined with intralesional triamcinolone acetonide injection for maxillofacial post-acne keloids

摘要： 目的：探讨浅层X线联合曲安奈德注射治疗颌面部痤疮后瘢痕疙瘩的有效性和安全性。方法：回顾性分析我院2022年4～12月浅层X线联合曲安奈德注射治疗的26例颌面部痤疮后瘢痕疙瘩患者的临床资料。三位独立的皮肤科医生评估疗效、复发及不良反应，采用Wilcoxon符号秩和检验对疗前疗后的配对数据进行假设检验t检验统计分析数据。结果：有7例患者失访，19例最终完成整个观察。治疗前VSS（vancouver scar scale）评分为(8.68±1.60)分，治疗后6个月VSS评分(4.68±1.10)分，差异有统计学意义（Z=-3.86,P＜0.01）；治疗1年后，复发3例，缓解率84.21%（16/19）。不良反应主要为放射线皮炎和色素沉着。结论：浅层X线联合曲安奈德注射治疗颌面部痤疮后瘢痕疙瘩安全有效。

Abstract: Objective: To access the efficacy and safety of superficial X-ray combined with triamcinolone acetonide injection in the treatment of maxillofacial post-acne keloids. Methods: A retrospective analysis was performed on the clinical data of 26 patients with maxillofacial post-acne keloids who received superficial X-ray therapy combined with triamcinolone acetonide injection in our hospital from April 2022 to December 2022. Three independent dermatologists assessed efficacy, recurrence and adverse reactions, and the Wilcoxon signed-rank test was employed for hypothesis testing on paired pre- and post-treatment data.the t-test was employed for statistical data analysis. Results: Seven patients were lost to follow-up and nineteen patients were completed the whole observation. The VSS score was 8.68±1.60 before treatment and 4.68±1.10 at 6 months after treatment, with a statistically significant difference (Z=-3.86, P＜ 0.01). One year after treatment, 3 cases relapsed, with a remission rate of 84.21% (16/19). The main adverse reactions were radiation dermatitis and pigmentation. Conclusion: Superficial X-ray therapy combined with triamcinolone acetonide injection is safe and effective for treating maxillofacial post-acne keloids.
